The Orbiter log is found in the same directory that you have Orbiter install in. Near the bottom it will usually tell you at which point the application turned south.

Having watched the video again, I would clearly see that the deacceleration burn did happen and it appears, as has been alluded too, that maybe an clean seperation wasn't achived, contributed to the upper dummy stage/payload to spin. At any rate, the test certianly verified what the engineer...

Basically it is a test of the first stage only. The rest is just ballast and will splash down in the Atlantic Ocean. The SRB will be recovered. Suborbital only.
