Let's see, "Errant Missile Locks on to Moscow" comes to mind...

...and subsequently splashes down in arctic ocean due to lack of fuel. Even if it landed in a random spot within 2500 miles (a number I pulled out of the ether), there isn't really much to hit in the middle of the pacific.

Edit: and it's not like it carrys a nuclear device. The two most dangerous things about it are any fuel it might have when it fails and crashes into the ocean 30 seconds after launch (unlikely), and it's kinetic energy.
