Maybe the water was deeper than expected. But they should have ended the flight there. Later it looks like the hanging aft stabilizer destroyed a rotor blade, making the coax rotor helicopter uncontrollable.
 
Ate Chuet (former Rafale-M pilot that comments the crash) thinks that the pilot had no time to go watch the tail state and was probably in a 'oops nothing happened there' mindset that is sadly not uncommon in aviation (these machines are expensive).

Highly possible. The view aft for the Ka-x26 family isn't the best.

Stumbled accidentally over this old OVA on youtube: Mighty Space Miners, originally planned as six episodes, unfortunately only two got made. So the story isn't concluded. But the two episodes that are there are really quite good!
